How it works: • Membership payments are made monthly. • Treatments are scheduled every 3 months (typical duration of wrinkle relaxer results). • Areas included in this membership: • Forehead lines • Frown/“11” lines (glabella) • Crow’s feet (eyes) • Brow lift • Jelly roll (under-eye) Important details & terms: • This membership covers upper face treatments only. • Dosage and results vary per individual; treatment plans will be customized at each appointment. • Any additional areas or units outside of the membership will be billed at the standard rate. • Treatments are provided at the discretion of the medical provider to ensure safety and effectiveness. • Membership is a 3-month minimum commitment and payments are non-refundable.
